!What Is Psoriasis at the Cellular Level? - SUSHENAH scientific illustration Psoriasis is a differentiation disorder—skin cells reach the surface before they've fully matured, with their nuclei still intact. According to research in Andrews' Diseases of the Skin, "A very thin or absent granular layer can lead to extensive parakeratosis in which the nuclei of keratinocytes persist as the cells move into the stratum corneum, resulting in psoriasis." Read that again. In healthy skin, cells pass through the granular layer where they lose their nuclei and become protective corneocytes. In psoriasis, this transformation doesn't complete. The visible patches—red, scaly, raised—are the surface result of a deeper disruption. This is why psoriasis isn't "just dry skin." It's not a moisturizer problem. Understanding this changes how you approach care. The answer isn't adding moisture to the surface—it's addressing the inflammatory cascade affecting cell maturation.
